- Photocopy of Land Title or Deed of Sale or Award or Certificate of Residency
- Original Copy of Meralco and Water Bill
- Picture of Business
- Photocopy of Business Permit
- Spouse Valid ID
- Sketch / Location Map
- Photocopy of Birth Certificate / Marriage Contract
- Proof of Remittance (if any)
- Submit the Original Certificate of Employment and latest Payslip (if employed)
